Holy Hierarch Hormisdas, Bishop of Rome (pope from 514 to 523) — put an end to the 35-year Acacian Schism between Rome and Constantinople, composing the "Formula of Hormisdas," a confession of faith signed by the Eastern bishops at the reunion of the Churches in 519.
Holy Hierarch Hormisdas was born in Frosinone, Italy, the son of a certain Justus; before his ordination he was married — his son later became Pope Silverius. He served as deacon under Pope Symmachus, supporting him through the years of his contested pontificate (498, 501-505).
He held the Roman see from July 20, 514, to August 6, 523. His pontificate is marked chiefly by resolving the 35-year Acacian Schism (which began in 484 over Rome's refusal to accept Emperor Zeno's "Henotikon," supported by Patriarch Acacius of Constantinople). Hormisdas composed the so-called "Formula of Hormisdas" — a confession of faith affirming Chalcedonian orthodoxy and anathematizing Nestorius, Eutyches, Dioscorus, and Acacius, while also affirming Rome's primacy in guarding orthodox teaching. After the accession of Emperor Justin I (518, a supporter of Chalcedon), the reunion was solemnly sealed in the cathedral of Constantinople before a large crowd on March 28, 519 — the Formula was signed by Patriarch John II of Constantinople and about 250 Eastern bishops, ending the schism.
